To save time at your initial visit with the doctor, please either:
1. Download and fill out the forms needed.
2. Print and fill out and bring the required forms with you to your first appointment.
Because the doctor stays on schedule, if you do not have your completed paperwork at the time of your appointment, your first visit may be delayed or rescheduled.
We accept two insurance plans for chiropractic. Our office offers a courtesy coverage check to verify how your insurance will support your care in the office. We currently are not accepting Medicaid or Medicare.
