We accept many insurance plans. Our office offers a courtesy coverage check to verify how your insurance will support your care in the office. We are currently not accepting Medicaid or Medicare.

Insurance plans that Dr.Jena is In-Network with are:

• Blue Cross and Blue Shield
• Priority Health

Please contact us if your plan is not on the list.

Many plans have restrictions and limitations for care, so the best thing to do is contact us, and we will get the necessary information from your insurance company.

The deductible, maximum number of visits, maximum dollar amount, and diagnosis restrictions are some of the questions that we inquire about to get the most accurate information about your specific plan.

We do the best we can to make the insurance side of your visit experience hassle-free as we do submit all necessary information to the insurance company, such as the billing, notes, and anything else they may need to process your claims accordingly.

To save time at your initial visit with the doctor, please either:

1. Download and fill the forms needed. 

2. Print and fill out and bring the required forms with you to your first appointment.

Because the doctor stays on schedule, if you do not have your completed paperwork at the time of your appointment, your first visit may be delayed or rescheduled.
